Hazard has long been linked with a move to the La Liga giants and appeared to say goodbye to Stamford Bridge last week.
It’s claimed the fee will be around €100million (£86m) with Real boss Zinedine Zidane keen to sign the winger.
Hazard revealed that he had made his decision after Chelsea’s final game of the Premier League season, which they drew 0-0 with Leicester.
The Belgian explained: “I have made my decision. I’m still waiting like you are waiting and like the fans are waiting.
“Yes, I wanted that [it cleared up earlier] but that’s not happened. I have told the club a couple of weeks ago.
When asked if Chelsea securing a top four finish made any difference Hazard explained: “No. I don’t think so. I have made my decision, that’s it. I’m still waiting.
“We have a final to play and then I will see.
“I have made my decision but it is not just about me.”
Hazard had been strongly linked with a move to Real Madrid last summer but stayed put at Stamford Bridge.
He has shone for the Blues this season and helped them to a third-placed finish in the Premier League.
Hazard celebrated wildly when Chelsea beat Eintracht Frankfurt on penalties to reach the Europa League final.
He joined his team-mates in celebration after the final whistle but took some time to reflect on his own and looked emotional as he clapped the adoring crowd.
The Belgium international also threw his shirt into the crowd in what appears to have been his final home game.
Chelsea may still try to keep hold of Hazard as they have been hit with a transfer ban for the next two windows.
Christian Pulisic will arrive at Stamford Bridge though after he was signed in January though loaned back out to Borussia Dortmund.
